Add an address to your professional healthcare credentials

Licensed professionals can add addresses to their Professional Healthcare credentials through the Licenses and credentials section in Business Settings.

To add an address to your Professional Healthcare credential:

1. Go to Business Settings.
2. Under Certifications, select Licenses and credentials.
3. Select your preferred credential.
4. Once you have completed the form, select Submit for verification.

Note:

• Professional-use products only ship to a verified address associated with your credential.
• Addresses added to your credentials must be commercial or nonresidential, and associated with your credential or work location. They must also be within the state of issue on the credential.
