Healthcare Purchasing and Disclosure Policy

Prescription Medical Devices; Applicable Professional Use Policy.

You may purchase prescription medical devices so long as you have all Credentials required by Amazon, which includes licenses provided under applicable laws and regulations. If you seek reimbursement for purchases you make under Medicare, Medicaid or other federal or state healthcare programs, you are responsible for complying with all federal or state laws, regulations, guidelines, and reporting requirements, including the following:

• You are responsible for accurately reporting any discounts or rebates to the appropriate healthcare agencies.
• You may have reporting obligations to federal healthcare agencies if items included in your order summary are subject to a later rebate.
• In preparing any documentation for the appropriate healthcare agency, you may be required to evaluate as a discount, for cost-reporting purposes, the value of any product listed as $0.00 on any invoice.
• You should not include as a discount, for cost-reporting purposes, the value of any item that is designated as a sample. You should not seek reimbursement for such items.

Information on discounts is available on the "Review your order" page at the time of purchase. You can also access the amount paid for each product by using Amazon Business Analytics reports. You can access these reports by selecting "Business Analytics" from the Account for Business dropdown menu. In the report, the "Item Net Total" column displays the price paid for all units of a particular product (inclusive of any discount and sales tax). For any given product, to calculate the net price paid per unit, you can divide the Item Net Total by the number of units purchased.
